Home Care Package Provider Stocktake - June 2021
The 2021 Home Care Package Provider Stocktake (for June 2021) is now available for completion via the online portal.
The Australian Government Department of Health (the department) is seeking information from Home Care Package (HCP) providers on the types, volume and cost of care and services delivered under the HCP Program in June 2021.

Purpose of the Stocktake
Information gathered through the Stocktake will provide greater insight into how the HCP Program operates and may inform workforce planning for the aged care sector and reform options for a new support at home program. In their Final Report, the Royal Commission into Aged Care Quality and Safety referenced the benefits that greater transparency of operations will provide the sector.
Who should participate in the Stocktake?
All approved HCP providers who are currently operating and provided care to people in June 2021. Participation is voluntary. However, the Department strongly encourages you to participate to ensure your circumstances are considered as part of the analysis.
How will the Stocktake be conducted?
The department has engaged Forms Administration to collect data on its behalf. Responses will be provided to the department in aggregate form. Feedback will be provided to the sector based on analysis of information received. The data collected will not be used for any compliance activity.
How long do I have to complete the Stocktake?
The Stocktake is open from 9am (AEST), Monday

Significant Refurbishment - Application for the Higher Accommodation Supplement
Significant Refurbishments - (Pre-Approval)
Use this form to apply for the higher level of accommodation supplement if you are an approved provider of a residential aged care service that has a proposed refurbishment project for which you are seeking pre-approval. Please note that if you complete the Pre-Application, you will also need to complete the Confirmation once refurbishment work is complete.
Significant Refurbishments - (Standard Application)
Use this form to apply for the higher level of accommodation supplement if you are an approved provider of a residential aged care service that has completed a refurbishment on or after 20 April 2012. Please note that if you complete the Standard Application, you do not need to complete the Pre-Application or the Confirmation.
Significant Refurbishments - (Confirmation)
Use this form to confirm that you have completed the significant refurbishment where pre-approval was given. Please note that you should only complete if you have previously completed the Pre-Approval Application and refurbishment work has been completed.
